Recent Aged Care Reforms have evoked mixed responses from older people. Tune D., (2017). Many are struggling to negotiate the My Aged Care Systems. During 2016, Country SA PHN (CSAPHN), received increased reports from service providers and general practices, identifying nursing care was impacted with more requests for assistance with navigating the My Aged Care Systems, (MAC). This raised an awareness in Country SA that a gap existed with practical support for older people in managing the processes to access the services they needed.
Late 2016, CSAPHN approached Council on the Ageing SA (COTA SA) discussed how, as a partnership, the issues could be addressed to support older people and the workforce in local communities.
COTA SA has a long history of peer education and is a known peak body and advocate for older people in SA. CSAPHN has established networks across regional South Australia (SA) which ensure that the gap in health equity is addressed and coordinated management of individuals needs is delivered in the right place and at the right time. CASAPHN data identified four regional areas where there is a higher than average ageing population. To this end, the pilot of My Aged Care Support Program was co-designed for delivery in the 2017-18 financial year.
